Gebruik van nystatine voor orofaryngeale candidiasis in het CHU van Luik

Summary
The antimicrobial stewardship team in the
university hospital of Liège emphasized
an important and inappropriate use of
mouthwash containing nystatin. We reviewed
the scientific literature on the efficacy
and safety of nystatin for oropharyngeal
candidiasis (OPC) and with this paper,
we would like to share some practical
institutional recommendations. OPC is a
common fungal infection with good prognosis
when appropriately diagnosed and treated.
The Infectious Diseases Society of America
(IDSA) recommends nystatin oral suspension
or nystatin pastilles as alternative treatment
for mild OPC. However, the evidence on the
efficacy and safety of nystatin for OPC is
sparse. In Belgium, nystatin is available in
oral suspension and in bulk powder. There
is no national consensus on formulation, nor
on dosage and duration of treatment with
nystatin for OPC. In our hospital, we decided
to stop the production of nystatin mouthwash
and to advocate the use of nystatin oral
suspension for mild OPC.
Samenvatting
De antibiotherapiegroep van het Universitair
Ziekenhuis van Luik vestigde recent de
aandacht op een veel voorkomend, maar
onrechtmatig gebruik van nystatine verwerkt
in een mondbad. Deze vaststelling leidde tot
een literatuurstudie naar de werkzaamheid en
veiligheid van nystatine voor oropharyngeale
candidose (OPC). In dit artikel vatten we de
resultaten hiervan samen, en delen we enkele
praktische aanbevelingen geformuleerd in het
ziekenhuis. OPC is een frequent voorkomende
schimmelinfectie met een goede prognose
wanneer ze correct gediagnosticeerd en
behandeld wordt. De Infectious Diseases
Society of America raadt nystatine aan,
verwerkt in een capsule of als drinkbare
suspensie, als alternatieve behandeling bij
milde OPC. Desondanks bestaat er weinig
literatuur over de werkzaamheid en de
veiligheid van nystatine voor OPC. Nystatine
is in België commercieel beschikbaar als
drinkbare suspensie en als vrac-product.
Er bestaat geen nationale consensus voor
wat betreft galenische vorm, dosering of
behandelingsduur met nystatine voor OPC.
In het UZ Luik werd besloten te stoppen met
de bereiding van een mondbad op basis van
nystatine en de voorkeur te geven aan een
suspensie van nystatine in het geval van
milde OPC.
